It was in the 1980’s that psychologists and doctors affirmed that babies and children feel pain. (Stay with me because I’m getting to the point)Psychology isn’t in a mature stage yet. In order to avoid being told simply that you'll have to increase your medication, you'll have to ask your therapist specific questions like, "what are some strategies for managing anxiety created by the need to meet others' expectations?" and "what are some therapeutic methods for addressing executive dysfunction?" You could explain how things would go, and what level of executive function issues you'd have, if you were painting a portrait solely for yourself, and then compare with what would happen if you were painting it for someone else. I would try talking to your therapist using concrete examples. It's also harder to be certain that what I'm doing will meet someone else's expectations than that it will meet my own, which makes me anxious and makes it harder to do things. For example, it's much harder for me to start a writing project for work than it is for me to start a personal writing project, because the work project has to be written to another person's specifications and so I have to approach it with another "layer" of analysis on top of the analysis I'm already doing. What you described sounds more like an executive function issue to me, specifically that awareness of others' expectations, and the need to perform in a way that will meet those expectations, decreases your executive functioning. I don't know if I have a PDA profile but I frequently have an instinctive, immediate feeling of "no, I don't want to do that" when asked to do things, even simple things like getting someone a glass of water, and my initial verbal response to a request or plan is often to point out the potential problems or reasons it can't be done the way the other person is suggesting. My understanding of pathological demand avoidance is that it's more about actively avoiding demands, for example by making excuses, or otherwise controlling situations in order to avoid or resist demands.
